If you are a new client at FABIC, or are making a referral on behalf of a new client, the best way to get started is to complete our initial intake form here.
We appreciate that the form is very detailed, but provides us with all the foundational information that we need to get started.
What funding options are available?
At FABIC Flagstone, we aim to make our clinical services as accessible as possible. You may be able to access funding through a range of options, including:
